Ship
A ship defect report starts at the vessel with a broken component and ends with a reported repair job. Between those two points it changes hands twice — to the office and back. This path follows that route and points at the documentation for each step.
At the vessel: recording the defect
- Creating a report — from the component in Maintenance with
Create Defect Report, or in the Ship Defects module withNew Defect Report. - The Details tab — component or title, defect category, failure date. A defect on a critical component is marked as such and given the highest priority by itself.
- The Root Cause tab.
- The Controls tab — every report needs an assigned Risk Assessment, and it has to belong to the component. Three ways to get one, plus the Management of Change.
- The actions — immediate, corrective and preventive. They are mandatory.
Worth knowing before you approve: five things have to be filled in, and afterwards nothing on board can be changed any more.
- Approving on board — only an approved report can be worked on in the office.
In the office: deciding and following up
- Handling in Office — edit, reject or delete. Rejecting keeps the report in the database, deleting does not.
- Reviewing Risk Assessment and Management of Change, plus the flag and class notifications.
- The follow-up process — where something still has to be clarified with the vessel. There is no limit to the number of steps.
Back to the vessel: preparing the closing
- Assigning job details — a new job or an existing one for the component. Without them the report cannot be closed on board.
- Approve & Resend to Ship for closing — this is where a newly created job actually comes into being, as a one-time job carrying SDR and a running number in its title.
At the vessel: closing it
- Closing the SDR on board — not in the Ship Defects module, but by reporting the maintenance job. Two routes lead there: the schedules list, or the component itself.
